The Dementia Care Connect Community is for subject matter experts (SMEs) in long term care. SMEs may include people who have advanced education in dementia care. This community will strive to share helpful resources and tools, identify best practices and lessons learned in dementia care, and share innovative ideas in dementia care.
To request to join, please click the “Request to Join” button and provide the following information:
• Name
• Email
• Organization Name
• Organization State
• Job role

After you click Request to Join, we recommend you select Real Time at the drop-down menu to select your email notification frequency for community discussions. This will ensure you receive timely notifications from the community discussions. You can adjust this frequency at any time by selecting My Profile, My Account, Community Notifications.
Please also note that if you have just now created your account, the request to join button should display within approximately one hour. You may need to refresh your screen at that time. There is a sync that runs at the top of the hour which will display the request to join button for new users.
